To send us a records request please click on one of the links below and fill out the information so we may process your request effectively.

Please note that a separate request will have to be filled out for each individual student. Form links are also separated by grade level according to corresponding counselor. Please be sure you are on the correct grade level form for the student you are requesting information or your request may be delayed.

We value student safety and privacy. We will verify school and official information before sending records.

You will be sent an enrollment confirmation form within 48 hours of submission. Please fill and send back to us for our reporting.
